Pre-admission by phone (for inpatients)
During the week before your admission, a representative from Patient Access Services may contact you. The purpose of this call will be to obtain:
- Information about insurance benefits and/or payment arrangements
- Current address
- Phone number
- Employer information (if any)
Depending on your insurance carrier, the representative may be able to answer your questions about co-payments or deductibles.
Financial counseling
If you do not have insurance coverage, Patient Access Services will refer you to a financial counselor. Contact financial counseling before your visit.
What to bring ?
You will need to have the following information with you when you check in:
- Valid Insurance card
- A form of payment for any co-pays or deductibles (credit card or checkbook)
- Valid passport
- List of prescriptions (you do not need the actual prescriptions unless requested by your doctor). Or fill out our medication list .
In addition, bring any medical devices or appliances you may need, including eyeglasses, hearing aids, dentures, canes, etc.

Ortho Spine & Sport Clinic does not specifically endorse any hotel, but we can give you recommendation for the hotels of your choices.
For patients who going surgical intervention Be sure to check that you have the Transfer Leter about where to go for your procedure.
